Stay connected with your cardiovascular care team for medication review and ongoing heart failure management.

New consultation $225 · Established follow-up $175

Understanding heart failure

Heart failure means the heart cannot pump enough blood to meet the body's needs; it does not mean the heart has stopped. Care often involves an ongoing plan, medications, and attention to changes in symptoms. A virtual follow-up complements, rather than replaces, examinations and testing when those are necessary.

What we’ll discuss

  • Changes in weight, swelling, breathing, or daily activity
  • Current medicines and how well you tolerate them
  • Hospital discharge instructions and existing care plans

Your ongoing care

Our team can review your history and treatment plan, discuss medication management, and coordinate with your primary care clinician or other specialists. New or worsening symptoms may require urgent in-person assessment rather than waiting for a scheduled virtual visit.
